I acknowledge that this course includes a mandatory face-to-face Practical Assessment, and am able to attend this in Tullamarine VIC (or at another pre-agreed location). 

I understand that this Practical should be booked and attended within 4 weeks of completing the online component, and that I may be withdrawn from the course with no refund due should I fail to do so.
